Dangerous Occurrence and Hazard Reporting

For all staff and students
A dangerous occurrence is any occurrence connected with ANU operations that could have caused death or serious injury to any person. Examples might include:

A limb from a tree fell on a car parked on ANU property causing considerable damage to the car;
A piece of scaffolding fell from a construction site on campus;
A visiting member of the public acts in an aggressive, violent and intimidatory manner to an ANU staff member; and
Insulation on a pipe catches fire as a result of a spark from oxy-welding.

The ANU procedure on Death, Injury, Exposure and Dangerous Occurrence Reporting requires dangerous occurrences be reported via the web-based OHS Incident Notification system at http://info.anu.edu.au/policies/Procedures/Human_Resources/OHS/Incident_Reporting.asp for legislative purposes, to identify the cause of the occurrence and for the development of prevention strategies.

A dangerous occurrence differs from a 'Hazard' as it is an event, that could have caused death or serious injury to any person, that did happen. A hazard is a potential risk that may develop into a dangerous situation. A hazard can be reported using a Hazard Report form at http://info.anu.edu.au/policies/Procedures/Human_Resources/OHS/Hazard_Reporting.asp

In accordance with the Occupational Heath and Safety (Commonweath Employees) Act 1991 the ANU is obliged to notify dangerous occurrences to Comcare, our OHS regulator. If you would like more information on Dangerous Occurrences please contact OHSIM@anu.edu.au

More efficient reporting and management of incidents and injuries on ANU campus

The ANU incident notification and supervisor statement forms have been revised to a more usable format with clearer communication of the report log and status (by email).

Outcome data will also become more readily available where an incident /injury has been recorded.
